Cardiologists have long relied on body mass index to flag patients at risk for heart disease. Yet a major new analysis shows that measure often misses the mark. The size of a person’s midsection offers a sharper signal. And the difference can mean the gap between reassurance and early intervention.
Researchers from the Cross Cohort Collaboration examined data on more than 260,000 adults tracked for an average of 20 years. They assessed nine cardiovascular outcomes ranging from heart attacks and strokes to heart failure, atrial fibrillation and mortality. The findings, published by the American College of Cardiology, reveal that waist circumference and waist-to-hip ratio reclassify risk in ways BMI alone cannot. Individuals with normal BMI but high central adiposity faced 15 to 50 percent greater risk across most outcomes.
Nearly one in five people with normal BMI showed elevated waist-to-hip ratio. Their risk climbed sharply. Among those classified as overweight, 40 percent carried high waist-to-hip ratios that signaled trouble. The pattern held even after accounting for traditional factors.
Visceral fat drives much of the danger. Stored deep around organs, this fat releases inflammatory compounds. It disrupts blood sugar control. It elevates blood pressure and alters cholesterol profiles. Subcutaneous fat beneath the skin lacks the same potent effects. The distinction explains why two people with identical BMI can face vastly different cardiac futures.
“Indeed, it appears that WC and WHR reclassify risk defined by traditional BMI thresholds,” said Michael J. Blaha, MD, MPH, senior author and director of clinical research at Johns Hopkins Ciccarone Center for the Prevention of Cardiovascular Disease. He pointed to people with normal weight yet elevated central adiposity who still registered higher risk.
Lead author Zeina A. Dardari, PhD, MS, emphasized the practical stakes. “Our findings emphasize the critical role of identifying elevated central adiposity, even in individuals with a normal BMI or with a BMI in the overweight range. Relying solely on BMI may result in misclassification of cardiovascular risk across a wide range of cardiovascular outcomes.”
Harlan M. Krumholz, MD, SM, MACC, FAHA, editor-in-chief of JACC, went further. “It is time to abandon a sole focus on body mass index. This enormously important study, based on data from hundreds of thousands of participants in large-scale cohort studies, authoritatively shows that waist circumference and waist-to-hip ratio provide critical information about cardiovascular risk, even among people with a BMI considered normal. Where fat is distributed matters, and these simple measures should be part of routine cardiovascular risk assessment.”
The study arrives months after related research reinforced the message. In March an analysis of nearly 2,000 African American adults from the Jackson Heart Study found excess belly weight predicted heart failure more strongly than BMI. Blaha, speaking to another outlet, reinforced the point. “It is becoming more recognized that BMI is not a great measure of metabolic and cardiovascular risk, and BMI is not a great measure of abdominal or visceral fat. Waist circumference and waist-to-hip ratio are good measures of abdominal/visceral fat. When added to BMI, they provide a much better assessment of body composition.” (Healthline)
Simple thresholds guide action. For women, a waist over 35 inches or waist-to-hip ratio above 0.85 counts as elevated. Men cross the line at 40 inches or a ratio greater than 0.90. Patients can measure at home with a tape. Measure waist at its narrowest point, hips at their widest, then divide. No lab test required.
Yet limitations remain. The primary analysis captured waist and hip data only once. It lacked details on diet, physical activity or genetic factors known to influence outcomes. Changes in fat distribution over time could alter trajectories. Still, the scale of evidence from hundreds of thousands of participants makes the core message hard to dismiss.
Cardiology practice stands at a pivot. Guidelines have leaned on BMI for decades because it is easy. Height and weight suffice. But ease comes at a cost when it mislabels risk for millions. Normal-weight individuals with hidden visceral fat slip through. Obese patients with low abdominal fat may receive unnecessary alarm.
Experts now call for routine waist measurement in primary prevention visits. It takes seconds. It costs nothing. It can reclassify patients into more accurate risk categories. That reclassification matters for decisions on statins, blood pressure therapy, lifestyle counseling and closer monitoring.
Mechanisms behind the risk gain clarity with each study. Visceral adipocytes behave like endocrine organs. They secrete cytokines that promote systemic inflammation. They impair insulin signaling. They contribute to endothelial dysfunction. The result is accelerated atherosclerosis, higher arrhythmia burden and greater strain on the heart muscle. Inflammation markers such as high-sensitivity C-reactive protein partially mediate the pathway to heart failure, as the Jackson Heart Study data showed.
Public health implications stretch wide. Obesity rates remain high across populations. Yet many with concerning metabolic profiles register as healthy by BMI alone. Shifting emphasis to central adiposity could identify at-risk adults earlier. It could also reassure those whose extra weight sits mostly in less harmful subcutaneous stores.
Prevention strategies follow the evidence. Balanced eating that limits saturated fat and refined sugar helps. So does regular exercise, adequate sleep and stress reduction. No single abdominal workout melts visceral fat preferentially. Total energy balance and consistent movement drive change. But knowing the target changes the conversation between doctor and patient.
The Cross Cohort Collaboration study, alongside earlier work, builds a compelling case. BMI retains value as a population screening tool. For individual risk assessment it falls short. Waist circumference and waist-to-hip ratio add precision that can save lives.
Clinicians who adopt these measures gain better insight. Patients gain clearer understanding of their own bodies. And the medical community moves one step closer to matching intervention to actual physiologic risk. The tape measure, it turns out, deserves a permanent place next to the scale.
